Benefit (A): Basic health screening package for two persons

A JCB Cardmember may purchase the promotional GHJ Basic Health Screening Package for two persons at a total price of RM999
(exclusive of applicable SST), provided that full payment is made using a JCB Card in one transaction.

The GHJ Basic Health Screening Package comprises:
(a) A basic health screening package for two persons. Details on the health check test list for each person is in Appendix A; and
(b) One-Night Hotel Accommodation for two persons. Terms and Conditions to enjoy this benefit as below.
For clarity, the normal price of the basic health screening package for one person is RM688 (exclusive of applicable SST)

Benefit (B): Inpatient Medical Package

A JCB Cardmember who is admitted as an inpatient at GHJ shall be entitled to a One-Night Hotel Accommodation
with a minimum medical bill of RM10,000 (exclusive of applicable SST).
Payment of all hospital charges incurred under this package shall be paid in a single transaction using a JCB Card.

Terms and Conditions Related to the One-Night Hotel Accommodation:
Subject to room availability and arrangements made by GHJ, the following terms and conditions apply:
(a) The following room types apply:
(i) Studio Room at Somerset Medini Hotel; or
(ii) Deluxe Room at OZO Medini Hotel,
(iii) A maximum of 2 persons per room applies. Additional persons are subject to each hotel’s fees and charges, and are to be paid by the Cardmember directly to the hotel

(b) Exclusions: The hotel accommodation
(i) Does not include breakfast;
(ii) Does not include additional requests such as room upgrade, additional night(s), meals, hotel room services, as well as transportation, incidental charges or additional services (such as laundry services) provided by the hotel
(iii) Any other additional requests shall be made directly with the relevant hotel, and all additional costs shall be borne by the JCB Cardmember.
(iv) Does not include Tourism Tax, which shall be payable by the Cardmember directly by the guest to the hotel;

(c) Use of Hotel Accommodation by Family Members or Accompanying Persons During a Cardmember’s Hospitalization
If a JCB Cardmember is hospitalised and could not stay at the hotel, the One-Night Hotel Accommodation may be utilised by the Cardmember’s family member(s) or accompanying person(s) during the Cardmember’s hospitalisation period. The person staying at the hotel is not required to be a JCB Cardmember.

(d) Use of Hotel Accommodation After Discharge
If the One-Night Hotel Accommodation benefit is not utilised during the Cardmember’s hospitalisation period, the Cardmember may carry forward and utilise it after discharge from the hospital, subject to both of the following conditions:
(i) the Hotel Accommodation benefit must be utilised within three (3) months from the Cardmember’s discharge date; and
(ii) Hotel Accommodation benefit must be utilised in connection with a medical-related visit to GHJ, such as a post-discharge follow-up appointment.
